Sutter Care at Home provides compassionate, personalized care in the comfort of a patient's home. Our visiting nurses, physical therapists, speech therapists, occupational therapists, medical social workers, dietitians, and other health professionals work closely with physicians to deliver exceptional, comprehensive home healthcare. Our ability to provide nationally-recognized quality care is possible not only because of our highly skilled clinical staff, but also because of the generosity of individuals who partner with us.
Please consider the impact your generosity can have to a Sutter Care at Home patient.
Hospice
Sutter Care at Home’s Hospice program provides physical, spiritual and emotional support focusing on pain management and symptom control, as well as comforting services and therapies. Contributions to Sutter Care at Home’s hospice program provide our patients and families with:
- Volunteer training to enter hospice circumstances with a deep level of sensitivity, offering respite to families.
- Professional training to ensure our hospice clinicians maintain best practices and stay up-to-date on the latest advancements in hospice care.
- Alternative therapies for our patients such as massage, guided imagery, aromatherapy, and music therapy.
Bereavement
The Sutter Care at Home Bereavement Program is available to those who use our hospice services as well as members of the local community. The program provides individual, group and school-based bereavement therapy services to more than 3,000 adults and 4,000 children each year.
Donor support enables Sutter Care at Home to offer its grief and bereavement services free of charge to all participants. Your gift to bereavement programs will expand our services, add additional staffing, and increase the number of community based clients and services we support.
Home Health
Our Home Health service line includes medical care for homebound patients, including home infusion therapy, curative treatment, home visits for rehabilitation, and speech, physical, and occupational therapy. Sutter Care at Home serves more than 39,000 Home Health patients each year.
Donations in support of Home Health increase Sutter Care at Home’s capacity to address the long-term needs of home health patients, including support for:
- Specialized care for specific needs (e.g., home infusion and pharmacy services for intravenous hydration, nutrition, and medication), including in-home nursing care for patients recovering from surgeries or serious illness.
- Equipment upgrades for home health nurses visiting patients in their homes, such as blood pressure cuffs, ultrasound machines, diabetes kits, physical therapy equipment and oxygen saturation monitors.
- Rehabilitation therapy services, including physical, speech, pulmonary and occupational therapies to help patients adapt to limitations and regain maximum function, mobility, and independence.
- Launching innovative new programs such as more proactive approaches to managing chronic illnesses, which has been shown to reduce ER visits, hospitalizations, and adverse health events among home health patients.
- Piloting digital technologies that can track key health indicators (e.g., heart rate, sleep patterns, glucose levels) to alert caregivers of potential problems before they progress.
Healthcare Heroes Fund
The generosity of donors to the Healthcare Heroes Fund will enable investments in programs and spaces that can help to alleviate the symptoms and causes of healthcare professional burnout, while also promoting wellness among all medical providers, staff, and patients across Sutter Care at Home. Donations to the Heroes Fund will address stress and burnout in the following ways:
- Gratitude and Recognition: Philanthropic funds will help provide respite activities during shifts and additional employee recognition through gratitude events.
- Staff Support and Development: Providing staff and trained volunteers with educational opportunities that give them the skills and tools they need to maintain their health. Opportunities may include speaker series, staff retreats and resources on reducing burnout.
