How Do I Schedule an Appointment?

After a PAMF physician has referred you to the Physical Therapy Department, you may call (408) 523-3060 to schedule an appointment. 

What Should I Expect at My First Appointment?

Please arrive at least 15 minutes prior to your scheduled time to check-in. At your appointment, a licensed rehabilitation therapist will review your doctor's notes and instructions, and complete an evaluation of your problem. The therapist will then design an individualized program to help you achieve your goals. This plan will be carried out by our team of experienced therapists and therapy assistants.

How Do I Get to My Appointment and Where Should I Park?

Physical therapy is provided at several locations: 582 South Sunnyvale Ave., and at 301 and 401 Old San Francisco Road in Sunnyvale. Parking is available in front of the buildings, on the street and in a PAMF parking lot located one block from 582 South Sunnyvale Ave.

How Long Will My Appointment Last?

Most therapy appointments are scheduled for 45 minutes, but your session may run longer.

What Insurance Is Accepted?

PAMF provides physical therapy services as appropriate to patients with HMO, POS and PPO insurance. Depending on your insurance, you may need a referral and authorization from your primary care physician prior to beginning physical therapy.

Patients should verify that services are covered by their insurance provider prior to receiving services.
