The Multidisciplinary Heart Failure Clinic offers teams of specialists, rather than a single doctor, to provide you with comprehensive care. We’ll work collaboratively with you from diagnosis to long-term management.

You’ll receive regular, interactive care from your primary care doctor, cardiologists, nurse coordinators, pharmacists, social workers and dietitians. Specialists remain in close contact with your primary doctor regarding your progress and any changes in your condition. If appropriate, you’ll also be screened for heart transplantation. In addition, trained telemanagement care nurses provide consultation, support and information while you remain in the comfort of your home.
Depending on the severity of your heart condition, you and your team may discuss a number of treatment options, including:
