When you and your family face serious illness, compassionate doctors, nurses, social workers and other specialists at CPMC hospitals and clinics can provide an extra level of care. We’ll coordinate with your primary care doctor and other caregivers to support you every step of the way.

We provide education and support for you and your loved ones. After a referral from your primary care provider, we’ll work closely with you and a nurse practitioner to develop a care plan based on your concerns, empowering you to better manage your condition. All of our providers are board-certified, and we serve hundreds of patients every year.

The Sutter Health network also offers Home Health and Hospice and Advanced Illness Management services for people recuperating from long-term illnesses or managing a chronic condition.

At CPMC’s Coming Home Hospice, end-of-life care for terminally ill patients and their families is provided in a peaceful, homelike environment.
