The Asthma Management Program at Palo Alto Medical Foundation is currently only available at our Santa Cruz location only. The program provides you the education and training you need to manage and prevent your asthma. Board certified asthma educators work closely with your doctors to develop asthma action plans designed to help you take control of your condition.

Services We Provide

Our program offers a wide range of asthma management services, including:

• An initial individual 60- to 90-minute appointment.
• Pulmonary (lung) function testing (when applicable, for patients 5 years of age and older).
• Ongoing support and individualized care.

Your Initial Visit

During your initial visit, you will receive:

• An explanation of asthma.
• Identification of your triggers and causes of asthma.
• Recognition of early warning signs.
• Review of your medication and equipment.
• An asthma action plan.
• Addressing your questions and concerns.
