Description
This project proposes to expand the pilot of Sutter Health’s Advanced Illness Management program (AIM) across the entire Sutter Health system in order to demonstrate that AIM:
(1) improves quality of care and patient/caregiver experience;
(2) reduces total cost of care; and
(3) can be implemented in a wide variety of health care settings with varying degrees of operational and financial integration.
