Hospice is a special way of caring for people with a life-limiting illness. Hospice provides physical, spiritual and emotional support, enabling you to live out your life with dignity and comfort, surrounded by the people and things you love. With hospice care, we emphasize supportive or palliative care. We focus on pain management and symptom control through medications, as well as supportive visits from your team.
Your dedicated hospice team includes spiritual support, specially trained nurses and home aides, doctors, medical social workers, bereavement experts and trained volunteers. All of our team is dedicated in providing compassionate care to you and your family. A personalized care plan is created in collaboration with you and your family, so we can focus on what is important to you.
You can reach hospice professionals 24 hours a day for phone consultations and after-hours emergency visits to respond to concerns and questions. In addition, we provide community education on end-of-life issues and community grief support.
Hospice is a philosophy of care provided wherever you call home, whether that is your home, an assisted living facility, nursing facility or hospice residential unit. We also offer grief and bereavement support for loved ones, often for up to 13 months.
