Reducing the Impact of Chronic Disease
San Francisco brothers Jonathan (age 12) and Adrian Mis Mis (age 8) overcame debilitating asthma thanks to HealthFirst, an innovative CPMC program funded in part by charitable gifts.
Trained community health workers, who live in the San Francisco neighborhoods HealthFirst serves, work with patients in the clinic and at home to help them better manage their chronic diseases. As a result, the Mis Mis boys reduced their medications and increased their overall energy and interest in school, while the family learned how to minimize environmental triggers at home.
