You should also call Anthem’s customer service number on your insurance card to let them know that you want to continue seeing your Sutter doctor.

Our contract with Anthem allows patients with Continuity of Care needs to continue seeing their Sutter doctor after Dec. 31, 2024, but only with Anthem’s prior approval. Please call the customer service number on your insurance card to request Continuity of Care.

Anthem must authorize certain Continuity of Care services. Please call the customer service number on your insurance card to request Continuity of Care for services such as:

1. Acute condition (e.g., pneumonia)
2. Serious chronic condition (e.g., severe diabetes or heart disease)
3. Pregnancy
4. Terminal illness
5. Care of a child under 3 years of age
6. An already scheduled surgery or other procedure (e.g., knee surgery or colonoscopy)
7. Healthcare services which have been authorized

You may access Sutter Health hospitals for emergency care only. Anthem would need to approve all other care under its Continuity of Care policies.
