How does the AIM Program Support Provider Referrals?

• Palliative Care Access Nurses (PCAN) support AIM eligibility validation on referral, with navigation to the right level of care, and cross-continuum care coordination

• Proactive identification of potentially eligible patients with provider engagement for timely enrollment for appropriate level of care.
