Can I still make an online payment if I do not know my Employee Number?
No. Your Employee Number is required to ensure your payment is applied to the correct account. If you need assistance determining your Employee Number, please contact the Sutter Health Employee Line at (855) 398-1631.
How will charges appear on my credit card?
Charges on your credit card statement will appear under the name of Sutter Benefit – LOA Prem.
How do I know how much I have to pay?
Please input the total amount you were charged as outlined on your most recent Premium Billing Statement.
When are premium payments due?
Premiums are due every two weeks, on regular pay days. Once you have made an online payment, make sure to notate the “Ticket Number” associated with your credit card payment for your records.
Can I pay premiums with my Wage Works FSA card?
No. Health Insurance / plan premiums cannot be paid with pre-taxed dollars.
How are overpayment refunds received?
Overpayments will be credited to the cardholder’s credit card that was originally charged.
What happens if I don’t pay my balance?
If payment is not received within the 15 days, you will receive a second notice. The second notice will be the final request for payment and will identify the retroactive termination date if full payment is not received. The termination date will be the date the premium was originally due. You may be responsible for repayment of claims incurred after the coverage termination date. If you return to work with an outstanding balance still due, you must pay this premium or coverage will be terminated even if current premiums are being deducted from your paycheck. If coverage is terminated due to non-payment of premiums, you and your covered dependent(s), if any, will not be eligible to continue coverage through COBRA. If terminated for non-payment of premiums you may enroll in benefits during the next open enrollment period. [Exception: If you are returning from a protected leave of absence (FMLA, PDL, CFRA ),coverage will be reinstated effective first of the month following election as long as you re-enroll and complete the enrollment process within 60 days of your return to work. This exception does not apply if you are returning from any other type of leave of absence.
