WHO CAN JOIN?
- Adults aged 18 and older.
- You must reside in California when the program starts to get a CGM prescription. If you're not in California, you can still join, with or without your own CGM.
- Note: We cannot enroll individuals with Type 1 diabetes, Type 2 diabetes on insulin, those who are pregnant / planning to be pregnant or those with chronic kidney disease on dialysis.
